Case Management
Hill Country Detox offers a highly individualized treatment focus that extends beyond physical treatment and into every aspect of case management, from comprehensive assessments and ongoing treatment plans to care coordination, individual evaluation, and family support. Every resident at our facility receives treatment from a coordinating multidisciplinary team that designs and facilitates a comprehensive care plan. This care strategy allows us to meet the unique needs and objectives of each client and their detox program.
During individual counseling sessions with a detox counselor, each client will continually discuss ongoing treatment options as they progress through their detox program. In addition to providing clinical support, detox counselors will also be in contact with family members to offer frequent progress reports on their loved one, make necessary medical appointments, and arrange for ongoing care and medical treatments such as Hepatitis C treatment, as needed.
As a whole, our case management process eliminates practical and logistical obstacles to make the detox process and the transition to continued care as stress-free as possible for our clients.
Discharge Planning
Clients each meet individually with their detox counselor for a discharge session. Recommendations and plans for ongoing addiction treatment are determined by the client’s treatment team, who consider a number of factors, such as:
- The client’s relapse history
- The client’s treatment history
- The client’s financial ability
- The client’s motivation for treatment
In many cases, the primary goal is to see clients through the transition into an inpatient rehabilitation center, but a client’s finances and willingness to go (among other circumstances) will ultimately determine the end outcome. In the event that a client does enroll in an inpatient treatment program, our clinical counselors will work with the facility’s admissions staff to oversee the transition.
